For the safety award, nominees will provide the following:
• A Copy of their OSHA 300 Logs for the Past Year Without the Name of Employees on the Form
• The Total Number of Recordable Cases (TRC) for the Past Year; the Rate Must be Below the National Average for the Company's Industrial Classification (SIC or NAICS)
• Total Number of Hours Worked During the Past Year
• Total Number of Work-related Injuries or Illnesses for the Past Year
• Total Number of Employees for the Past Year
• Experience Modification Information for the Past Year
• Historical Data for the Past Year - this includes occurrence of workplace fatalities, serious workplace accidents, and citations for serious violations 
• Open Anti-Discrimination (UALD) and Industrial Accident Cases
• Provide a Description of the Business - please include SIC or NAICS codes
• Briefly Outline Safety Efforts – especially with multi-lingual workers
• How do the Nominee's Employees Support Safety Efforts?
